By completing an Access to Support Form, a Request for Counselling Form, a You in Mind Support Request form or any other form for accessing the support offered by Mind BLMK through this website, you give permission to Mind BLMK to process your information in line with the Data Protection act 1988. You agree that information can be both sought and shared with Mind BLMK about your health and welfare and that such information will be securely stored, both on paper and electronically.
