This joined-up approach offers non-clinical intervention, which bridges the clinical treatment bringing professionals together, and working in tandem to provide the best outcomes for people.

Services Working in Partnership

Community Connectors collaborative provision within NHS mental health teams.

  • Mind BLMK work as part of blended teams with the Community Mental Health Teams across Bedfordshire and Milton Keynes.
  • We provide a Face-to-Face service offering one-to-one support for people with enduring and severe mental health challenges.
  • We support people to get back into the community and reduce isolation. This could mean supporting them to attend services and groups within their local areas.
  • This service is only available to those under the NHS Community Mental Health Team. If you require support from the Community Mental Health Team, please contact your GP to refer to this NHS provision. Referrals cannot be made via our referral form on the website.

What the Service provides:

  • Work within a Primary Care Network as part of a blended mental health team working individually with people to co-produce plans and set goals.
  • Take a holistic approach – looking at wider determinants of health.
  • Offer an in-depth knowledge of the local VCSE and community resources.
  • Support people on a 1:1 basis over 12 weeks to overcome barriers to engagement and walk the journey together.

Community Navigators collaborative provision within the Single Point of Access team:

  • Mind BLMK work as part of the Milton Keynes NHS mental health service. Working alongside colleagues from Central Northwest London Foundation Trust (CNWL).
  • We provide referral management to and from secondary services. This includes signposting patients to statutory services including talking therapies. Mind BLMK peer support and recovery groups and wider community-based & voluntary sector support.

What the service provides:

  • Within the Single Point of Access (SPA) based at Milton Keynes Hospital as part of a blended mental health team, respond to calls from people with mental health issues and diagnosis, to offer triage and support to them and their families.
  • Listen empathetically and non-judgementally to individual’s concerns and needs and signpost accordingly. This could include Mind BLMK crisis cafes, peer support and recovery groups as well as wider community-based & voluntary sector support.
  • Provide in-depth information on clinical and non-clinical support and community-based resources.
